According to the study \u2018Does Blood Pressure Inevitably Rise With Age?\u2019, published originally in the journal <em>Hypertension<\/em> in May 2012, in Westerners over the age of 40 years old, blood pressure increases <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/HYPERTENSIONAHA.111.189100\" target=\"_blank\" rel=\"noopener\">by around 7 mm\/Hg per decade<\/a>. Systolic blood pressure is also noted to increase, and by the eighth decade, is around 140 mm\/Hg.<\/p>\n\n\n\n<p>Caring for an older person or a child places a lot of responsibility on your shoulders, and it\u2019s important to be aware of when they might be at risk of developing high blood pressure. One thing you might not understand is the natural fluctuation of blood pressure during the day. These fluctuations may trick you into believing your loved one is suffering from high blood pressure or cause you to fail to recognise they\u2019re at risk of high blood pressure.<\/p>\n\n\n\n<p>Below is a look at the nature of blood pressure fluctuation, how blood pressure fluctuations can be misleading, some of the questions around blood pressure medication, and how to care for someone in terms of their blood pressure and taking blood pressure readings.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The nature of blood pressure fluctuation<\/h2>\n\n\n\n<p>Over the course of a 24 hour day, several of the body\u2019s functions vary. Blood pressure circadian rhythm is one of them.<\/p>\n\n\n\n<p>Normally, blood pressure will rise a few hours before we wake and will then continue to rise until around midday, which is when it\u2019s at its highest. Blood pressure tends to fall during the late afternoon and the evening. At night it tends to be lower. This is the typical pattern.<\/p>\n\n\n\n<p>Examples of an <strong>irregular blood pressure pattern<\/strong> include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>high blood pressure during the night;<\/li>\n\n\n\n<li>high blood pressure early in the morning;<\/li>\n\n\n\n<li>a drop in blood pressure of less than 10% overnight (non-dipping blood pressure).<\/li>\n<\/ul>\n\n\n\n<p>Irregular blood pressure patterns could mean the person you\u2019re caring for has:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>badly controlled high blood pressure;<\/li>\n\n\n\n<li>obstructive sleep apnoea;<\/li>\n\n\n\n<li>kidney disease;<\/li>\n\n\n\n<li>diabetes;<\/li>\n\n\n\n<li>thyroid disease;<\/li>\n\n\n\n<li>a nervous system disorder.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Misleading fluctuations in blood pressure<\/h2>\n\n\n\n<p>It\u2019s likely you already know the common factors that affect blood pressure, such as age, obesity, lack of exercise, poor diet and genetic history; however, there are other factors you may not be aware of, such as the circadian rhythms, which we\u2019ve already touched upon and which create natural fluctuations in blood pressure. Here are some more:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Labile hypertension<\/h3>\n\n\n\n<p>Labile hypertension refers to blood pressure that fluctuates far more than normal. Blood pressure rises and falls various times during the day. It can even do so within minutes, but if their blood pressure changes excessively within minutes, this could be a sign your loved one is suffering from labile hypertension. Physical activity, emotion, body position, diet and sleep deprivation are all potential factors.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Whitecoat hypertension<\/h3>\n\n\n\n<p>Do you have to take your loved one to the doctor\u2019s surgery or other medical facility? If they show high blood pressure readings in the doctor\u2019s office but regular readings in nonmedical settings, they could be suffering from \u2018whitecoat hypertension\u2019. Some people find medical visits stressful, and the stress triggers a temporary increase in blood pressure. Research suggests untreated whitecoat hypertension <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31181575\" target=\"_blank\" rel=\"noopener\">carries an increased risk of heart problems<\/a>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Masked hypertension<\/h3>\n\n\n\n<p>Some people experience normal blood pressure readings at the doctor\u2019s office but high ones at home, a phenomenon called masked hypertension and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5733331\" target=\"_blank\" rel=\"noopener\">is estimated to occur in 10 to 30% of people<\/a>. No one really knows how common this is because when a patient gives a normal blood pressure reading in the surgery, the doctor doesn\u2019t tell them to monitor their blood pressure at home.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Understanding about medicines for blood pressure<\/h2>\n\n\n\n<p>If you\u2019re caring for someone who is taking medication for their blood pressure, it\u2019s important to know what this medication is and how it works. Be sure to keep a diary of the brand names and generic names, and the dosages.<\/p>\n\n\n\n<p>Your loved one should take the medicines as scheduled, at the same time every day. If either you or they think they need a change in medication, speak to a doctor first. Avoid abruptly taking them off their medication if you think they need to come off it. Again, visit or discuss with the doctor before taking action.<\/p>\n\n\n\n<p>Keep a medication calendar, and record every time your loved one takes a dose of their medication. Although prescriptions inform patients how much to take in each dose, doctors change doses regularly, depending on the patient\u2019s response to the drug. In addition, the research showed there was no meaningful difference in hospitalisation for heart attacks, strokes or vascular death between the people who took their medication in the morning and those who took it in the evening.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Changing the time of the dose<\/h3>\n\n\n\n<p>Although it\u2019s becoming acknowledged that a patient can take their medication at the time that suits them, and that there are findings indicating the time of day doesn\u2019t affect the impact of the dose, a patient should take their medication at the same time each day. If they\u2019re taking it at night-time, there\u2019s no reason to change suddenly or vary the times. It is important to maintain the routine as prescribed so the physician and you can have an accurate idea of the effects of taking the medication at a certain time of day. If you decide they should change the time of their dose, speak to their doctor first for advice.<\/p>\n\n\n\n<p><strong>Losartan and lisinopril<\/strong><\/p>\n\n\n\n<p>If you are taking a medication such as losartan or lisinopril, and wonder what is the best time to take the medication, studies have shown little difference in taking the medication at a specific time of day. It\u2019s much more important to take the medication regularly at the same time of the day. If your loved one takes it in the morning, then make sure they take it in the morning every day. If, after consultation with your physician, the plan is to change the time to take it in the evening, then ensure they start taking it in the evening, at the same time, every day.<\/p>\n\n\n\n<p>Often people are taking medications for cholesterol and blood pressure at the same time. If your loved one experiences dizziness, fainting, nausea or complains of blurred vision, take them to the doctor.
